ABSTRACT Objective: To investigate the effects of orthodontic tooth movement on clinical attachment level (CAL) changes in treated periodontitis in adult patients with malocclusion. Material and Methods: Present study is based on PRISMA guidelines; all articles published in international databases such as PubMed, Scopus, Science Direct, and Embase between 2012 to May 2022 are included. 95% confidence interval (CI) for mean difference with fixed effect modal and inverse-variance were calculated. Data analysis was performed using STATA.V16 software. Results: In the initial review, duplicate studies were eliminated, abstracts of 175 studies were reviewed, two authors reviewed the full text of 21 studies, and finally, eleven studies were selected. The mean of CAL gain was 2.29 mm (MD, 95% CI -2.47 mm, -2.12 mm; p=0.00) (I2=91.81%; p=0.00; high heterogeneity). The mean difference of PPD changes was -1.93 mm (MD, 95% CI -2.07 mm, -1.80 mm; p=0.00) (I2=98.52%; p=0.00; high heterogeneity). Conclusion: Due to the limitations of the study and based on the meta-analysis, it is observed that orthodontic treatment is performed with higher success after reconstructive surgery with periodontal improvement.

ABSTRACT

Abstract Objective: To evaluate the effectiveness of Antimicrobial Photodynamic Therapy (aPDT), associated with scaling and root planing in the non-surgical periodontal treatment of individuals with Down Syndrome. Material and Methods: A controlled, randomized, split-mouth study was conducted. A total of 8 participants diagnosed with Down Syndrome aged 17-38 years of both sexes with clinical periodontitis were included in the study. Participants were treated at least three times: at the baseline, Plaque Index (PI), Bleeding on Probing (BOP), and Probing Pocket Depth (PPD) were obtained. After one week, conventional scaling and root planing were performed, and two quadrants were randomly selected for aPDT application. The reassessment was obtained one month after the aPDT application. The significance level was set at 5%. Analyses were performed considering a 95% confidence interval. Results: In the intergroup evaluation, no statistically significant differences were observed (p>0.05). In the intragroup evaluation, no statistically significant variations were observed in relation to the PI (p>0.05) and PPD (p>0.05); however, a statistically significant reduction in the BOP was observed between the test group (p=0.013) and control group (p=0.015). Conclusion: The use of aPDT as adjuvant therapy did not promote additional benefits in decreasing PI and PPD after 1 month of treatment. However, a significant reduction in the BOP was observed in the intragroup evaluation.

ABSTRACT

A doença periodontal (DP) e a diabetes mellitus (DM) possuem uma relação bidirecional bem estabelecida uma vez que compartilham mecanismos inflamatórios crônicos semelhantes. Por seus efeitos microbianos e imunológicos, os probióticos têm sido sugeridos como adjuvantes ao tratamento destas duas condições tanto de forma profilática (preventiva) como terapêutica (durante a doença). Sendo a metformina (MET) o fármaco padrão para o tratamento da diabetes, o objetivo deste estudo foi avaliar a eficácia anti-hiperglicemiante, anti perda óssea e anti-inflamatória do probiótico Lactobacillus rhamnosus EM1107 em modelo experimental de periodontite induzida por ligadura em ratos diabéticos tratados com metformina. Este ensaio pré-clínico, in vivo, randomizado, cego e controlado foi composto por 114 ratos Wistar machos divididos aleatoriamente em seis grupos com 19 animais cada: controle, ligadura, ligadura/DM, ligadura/Prob, ligadura/DM/Prob e ligadura/DM/Prob/Met. Durante 30 dias os animais receberam probiótico via gavagem oral. A DM foi induzida no 14º dia do experimento com injeção de Streptozotocin (STZ) na veia peniana, seguida pela colocação de ligadura para indução da DP e gavagem de MET no 19º dia, e eutanásia no 30º dia. Observou-se uma redução extremamente significativa da glicose sérica tanto nos grupos Prob quanto no grupo Metformina (p<0,001) pela análise bioquímica. A análise microtomográfica (µCT) do osso alveolar e da área de furca nas maxilas evidenciou que os animais diabéticos tratados com probiótico e com metformina tiveram perda óssea linear e volumétrica significativamente menor do que aqueles que não tiveram tratamento algum. Pela análise histomorfológica (HE), todos os grupos que receberam probiótico tiveram uma redução do infiltrado inflamatório, além de uma preservação de cemento e do osso alveolar. A análise pelo TRAP revelou que o grupo Ligadura/DM/Prob/Met apresentou a maior redução na contagem de osteoclastos ativos (p<0,01). Observou-se redução significativa de IL-1ß e TNF-α nos grupos Ligadura/Prob (p<0,05) e Ligadura/DM/Prob (p<0,01) por meio do ensaio ELISA em tecido gengival. Nossos achados sugerem eficácia anti-hiperglicemiante, anti perda óssea e anti-inflamatória do probiótico Lactobacillus rhamnosus EM1107 admnistrado tanto isoladamente quanto em associação com a Metformina na DM associada a DP (AU).


Periodontal disease (PD) and diabetes mellitus (DM) have a well-established bidirectional relationship in the literature as they share similar chronic inflammatory mechanisms. Due to its microbial and immunological effects, probiotic therapy has been suggested as an adjuvant to non-surgical periodontal treatment. Considering metformin (MET) as the standard drug for the treatment of diabetes, the aim of this study was to evaluate the antihyperglycemic, anti-bone loss and anti-inflammatory efficacy of the probiotic Lactobacillus rhamnosus EM1107 in an experimental model of ligature-induced periodontitis in diabetic rats treated with metformin. This pre-clinical, in vivo, randomized, blinded and controlled trial consisted of 114 male Wistar rats randomly divided into six groups of 19 animals each: control, ligation, ligation/DM, ligation/Prob, ligation/DM/Prob and ligation /DM/Prob/Met. For 30 days the animals received a probiotic via oral gavage. DM was induced on the 14th day of the experiment with injection of Streptozotocin (STZ) into the penile vein, followed by ligation for PD induction and MET gavage on the 19th day, and euthanasia on the 30th day. An extremely significant reduction in serum glucose was observed in both the Prob and Metformin groups (p<0.001) by biochemical analysis. Microtomographic analysis (µCT) in the maxilla showed that diabetic animals treated with probiotic and metformin had significantly lower linear and volumetric bone loss than those that had no treatment at all. By histomorphological analysis (HE), all groups that received probiotics had a reduction in inflammatory infiltrate, in addition to preservation of cementum and alveolar bone. The TRAP analysis revealed that the Ligature/DM/Prob/Met group had the greatest reduction in osteoclast count (p<0.01). There was a significant reduction in IL-1ß and TNF-α in the Ligature/Prob (p<0.05) and Ligature/DM/Prob (p<0.01) groups by means of the ELISA assay in gingival tissue. Our findings suggest the anti-hyperglycemic, antibone loss and anti-inflammatory efficacy of the probiotic Lactobacillus rhamnosus EM1107 administered either alone or in association with Metformin in DM with PD (AU).

Introdução: A ocorrência de multimorbidade, a qual corresponde ao acúmulo de doenças crônicas, é considerada um problema de saúde pública e suas consequências na saúde bucal carecem de evidências científicas em grandes populações. Objetivo: Avaliar o impacto da multimorbidade nas condições de saúde bucal em idosos brasileiros. Método: Realizou-se um estudo transversal e de base populacional, utilizando a base de dados da Pesquisa Nacional de Saúde. A presença de multimorbidade em idosos foi considerada quando o mesmo possuía um diagnóstico de duas ou mais doenças crônicas. Com relação às condições de saúde bucal, essas foram coletadas a partir das variáveis estudadas na referida pesquisa. O teste Qui-quadrado foi utilizado para a análise dos dados e em seguida as razões de prevalência foram ajustadas por meio da regressão múltipla de Poisson. Resultados: Participaram 11.697 idosos e, desses, 53,1% possuíam multimorbidade. Na análise multivariada, observou-se que a presença de multimorbidade em idosos predispôs a um relato negativo da autopercepção de saúde bucal (p=0,025), à dificuldade de se alimentar devido a problemas dentários (p0,001), a perder totalmente os dentes superiores (p0,001) e a escovar os dentes ou próteses pelo menos uma vez ao dia (p=0,025). Conclusão:Diante dos resultados, conclui-se que a maioria das condições de saúde bucal, assim como a autopercepção das mesmas apresentam-se piores na presença de multimorbidade na população idosa (AU).


Introduction:The occurrence of multimorbidity, which corresponds to the accumulation of chronic diseases, is considered a public health problem and its consequences on oral health lack scientific evidence in large populations.Objective:To evaluate the impact of multimorbidity on oral health conditions in elderly Brazilians.Methods:A cross-sectional and population-based study was carried out, using the database of the National Health Survey. The presence of multimorbidity in the elderly was considered when they had adiagnosis of two or more chronic diseases. Regarding oral health conditions, these were collected from the variables studied in the National Health Survey. The Chi-square test was used for data analysis and then the prevalence ratios were adjusted using Poisson multiple regression.Results:11,697 elderly people participated and, of these, 53.1% had multimorbidity. In the multivariate analysis, it was observed that the presence of multimorbidity in the elderly predisposed to a negative report of self-perceived oral health (p=0.025), the difficulty of eating due to dental problems (p0.001), to lose teeth completely (p0.001)and brushing teeth or dentures at least once a day (p=0.025). Conclusions:In view of the results, it is concluded that most oral health conditions, as well as their self-perception, are worse in the presence of multimorbidity in the elderly population (AU).

Objetivo:Evaluar el impacto de la multimorbilidad en las condiciones de salud bucal en los brasileños de edad avanzada. Método:Se realizó un estudio transversal y basado en la población, utilizando la base de datos de la Encuesta Nacional de Salud. Se consideró la presencia de multimorbilidad en los ancianos cuando tenían un diagnóstico de dos o más enfermedades crónicas. En cuanto a las condiciones de salud bucal, se obtuvieron de las variables estudiadas en el Encuesta Nacional de Salud. La prueba de Chi-cuadrado se usó para el análisis de datos y luego las razones de prevalencia se ajustaron mediante regresión múltiple de Poisson. Resultados:Participaron 11.697 personas mayores y, de estos, 53.1% tenían multimorbilidad. En el análisis multivariante, se observó que la presencia de multimorbilidad en los ancianos predispone a un informe negativo de autopercepción de la salud bucal (p=0,025), la dificultad de comer debido a problemas dentales (p0,001), para perder los dientes por completo (p0,001) y cepillarse los dientes o las dentaduras postizas al menos una vez al día (p=0,025).Conclusiones:En vista de los resultados, se concluye que la mayoría de las afecciones de salud bucal, así como su autopercepción, son peores en presencia de multimorbilidad en la población de edad avanzada (AU).

ABSTRACT Objective: To analyze periodontal comparison between Systemic Lupus Erythematosus (SLE) subject and healthy control. Material and Methods: This descriptive cross-sectional study included 122 subjects, 61 SLE patients and 61 healthy subjects who visited the Rheumatology Department, Dr. Saiful Anwar General Hospital, Malang, during 2017-2018. Clinical examination of SLE was using Mexican SLE Disease Activity Index and oral cavity conditions were assessed using the periodontal index, gingival index, calculus index, bleeding on probing, clinical attachment loss and mobility teeth. Results: The age of SLE patients ranged from 18-55 years old with the mean age of 29.50 ± 9.57 years old. Periodontitis was higher in SLE patients (88.5%) than healthy subjects (22.95%). In addition, periodontitis occurrence in SLE (2.66 ± 1.02) was significantly different (p<0.001) compared to healthy subjects (0.51 ± 0.81). Conclusion: This study found higher rates of periodontitis, gingivitis, bleeding on probing, clinical attachment loss, and mobility tooth among SLE patients compared to healthy subjects. Periodontitis was also found to be higher along with more severe SLE group.

Abstract Objective: To measure the level of immunoglobulin G (IgG) in the gingival crevicular fluid (GCF). Material and Methods: A total of 158 patients aged >45 years were examined for periodontitis and interviewed regarding their menopausal status. The non-menopause group entailed female patients with periodontitis without menopause (n=23). The menopause group included females who stopped menstruating since >1 year, had a pocket depth of 4-5 mm, and did not have other systemic conditions (n=40). Samples were selected based on periodontal and menopausal status. In total, 63 samples of GCF were collected from the participants and tested using an enzyme-linked immunosorbent assay kit for IgG. Results: The median level of IgG in the menopause group was 39.50 (g/mL, whereas that of the non-menopause group was 41.08 (g/mL. There was a positive correlation between the plaque index and IgG level in both groups. In contrast, there was a negative correlation between age and IgG level. However, there was no correlation between plaque index and age regarding the IgG level in both groups (p>0.05). Conclusion: The IgG levels in the menopause group were lower than those in the non-menopause group. As such, menopausal females should take great care of their overall health, including the periodontium.

Periodontal disease is the most common infectious disease that occurs in feline patients. Although it occurs in both sexes, different age groups, and any breeds, the prevalence and severity seem much higher in cats living in animal shelters. This paper aimed to describe the clinical, radiological, cytopathological, and virological aspects of periodontal disease and its complications in cats, based on these aspects and, consequently, on the importance it brings to cat feline medicine in shelter cats. For this, nine cats with periodontal disease from a single animal shelter were evaluated. These cats demonstrated a disease characterized by halitosis, excessive salivation, and oral discomfort. Lymphadenomegaly of the mandibular and retropharyngeal lymph nodes was observed in 44.4% of the cases. Oral lesions consisted of varying degrees of gingival hyperemia, complete loss of free gingival margins, and consequently gingival retraction, dental calculus deposition, dental mobility, complete exposure of the furcation of premolars and molars, and dental roots of canines and incisors, loss of bone radiopacity due to alveolar bone resorption and tooth loss. Complications included chronic ulcerative paradental stomatitis (22.2%), faucitis (22.2%), and chronic gingivostomatitis (11.1%). None of the cats affected by periodontal disease was positive for FIV or FeLV. In 33.3% of the cases, cats were carriers of feline calicivirus, but not feline herpesvirus.(AU)


Doença periodontal é a mais comum doença infecciosa que ocorre em pacientes felinos. Embora ocorra em gatos de ambos os sexos, diferentes faixas etárias e quaisquer raças, a prevalência e a gravidade parece muito maior em gatos que vivem em abrigos para animais. Baseado nesses aspectos e, consequentemente, na importância que ela traz para a medicina felina de gatos de abrigos, o objetivo desse artigo é descrever os aspectos clínicos, radiológicos, citopatológicos e virológicos da doença periodontal e suas complicações em gatos. Para isso, nove gatos com doença periodontal oriundos de um único abrigo de animais foram avaliados. Esses gatos demonstraram uma doença caracterizada por halitose, salivação excessiva e desconforto oral. Linfadenomegalia dos linfonodos mandibulares e retrofaríngeos foi observada em 44,4% dos casos. As lesões orais consistiam de graus variados de hiperemia gengival, perda completa das margens gengivais livres e, consequentemente, retração gengival, deposição de cálculo dental, mobilidade dentária, exposição completa da furca dos pré-molares e molares e das raízes dentárias dos caninos e incisivos, perda de radiopacidade óssea devido à reabsorção de osso alveolar e perda dentária. Complicações incluíram estomatite paradental ulcerativa crônica (22,2%), faucite (22,2%) e gengivoestomatite crônica (11,1%). Nenhum dos gatos afetados pela doença periodontal foi positivo para FIV ou FeLV. Em 33,3% dos casos, os gatos eram portadores do calicivírus felino, mas não do herpesvírus felino.(AU)

A microbiota e o sistema imune do idoso apresentam algumas alterações, favorecendo ao aparecimento de infecções e doenças inflamatórias. A doença periodontal é um exemplo, permeando entre fase imediata e tardia, pode ter alterações em sua evolução com o envelhecimento humano. Compreender a doença periodontal e sua relação com o ciclo da vida é importante para a prevenção, tratamento e cura. Este estudo tem como objetivo avaliar a quantidade de mastócitos (triptase), células dendríticas imaturas (CD1a), células dendríticas maduras (CD83) e vasos sanguíneos (CD34) em 154 tecidos periodontais saudáveis e doentes (27 idosos e 127 adultos). Foi utilizada a técnica de imunoistoquímica através da imunomarcação do CD1a, CD83, triptase e CD34, sendo contabilizados em 5 campos de maior número de células positivas, no aumento de 1000x. Para o CD34, ainda foram calculadas a área e o perímetro microvascular para todos os vasos sanguíneos presentes, e dos vasos com presença do endotélio vascular alto. Não houve diferença na imunomarcação das células dendríticas, dos mastócitos e na quantidade de vasos sanguíneos nos tecidos gengivais, entre os casos de gengiva clinicamente saudável, gengivite induzida por biofilme e periodontite estágios II, III e IV, avaliando isoladamente os grupos etários: adultos e idosos. As células dendríticas imaturas são mais numerosas no idoso com o quadro clínico de gengivite e periodontite. Os adultos com gengivite induzida por biofilme possuem maior quantidade de vasos sanguíneos que o grupo idoso. A área microvascular e o perímetro microvascular dos vasos sanguíneos com o endotélio vascular alto apresentaram maiores nos idosos nos casos de gengivite. Este estudo concluiu que nesta amostra não houve diferença na quantidade de células dendríticas imaturas e maduras, mastócitos na doença periodontal dentro dos grupos etário, porém as células dendríticas imaturas estão mais presentes no idoso podendo estar relacionado a algum decréscimo funcional. Em relação aos vasos sanguíneos, há presença de HEVs em adultos e idosos, não havendo diferença entre os diagnósticos. Nos idosos com gengivite há um aumento da área microvascular e perímetro microvascular, necessitando de estudos que justifiquem esta diferença (AU).


The elderly's microbiota and immune system show some changes, favoring the onset of infections and inflammatory diseases. Periodontal disease is an example, permeating between immediate and adaptative stages, it can have changes in its evolution with human aging. Understanding periodontal disease and its relationship with the life cycle is important for prevention, treatment and cure. This study aims to assess the amount of mast cells (tryptase), immature dendritic cells (CD1a), mature dendritic cells (CD83) and blood vessels (CD34) in 154 healthy and sick periodontal tissues (27 elderly and 127 adults). The immunohistochemistry technique was used through the immunostaining of CD1a, CD83, tryptase and CD34, being counted in 5 fields with a greater number of positive cells, in the 1000x increase. For CD34, the microvascular area and perimeter were also calculated for all blood vessels present, and for vessels with the presence of high vascular endothelium. There was no difference in the immunostaining of dendritic cells, mast cells and the amount of blood vessels in the gingival tissues, between cases of clinically healthy gingiva, biofilm-induced gingivitis and stages II, III and IV periodontitis, evaluating the age groups: adults and elderly. Immature dendritic cells are more numerous in the elderly with the clinical picture of gingivitis and periodontitis. Adults with biofilm-induced gingivitis have a greater amount of blood vessels than the elderly group. The microvascular area and the microvascular perimeter of the blood vessels with the high vascular endothelium were larger in the elderly in cases of gingivitis. This study concluded that in this sample there was no difference in the amount of immature and mature dendritic cells, mast cells in periodontal disease within the age groups, however, immature dendritic cells are more present in the elderly and may be related to some functional decrease. Regarding blood vessels, there are HEVs in adults and the elderly, with no difference between diagnoses. In the elderly with gingivitis there is an increase in the microvascular area and microvascular perimeter, requiring studies that justify this difference (AU).

Abstract Objective: To determine the diversity of microbiota in the sub-gingival fluids from periodontitis and rheumatoid arthritis (RA) patients and to know the relationship of microbiota abnormalities between periodontitis and RA. Material and Methods: Samples were collected from sub-gingival fluids of nine participants by swabbing technique. The participants were divided into 3 groups, namely the control (not periodontitis and RA), periodontitis, and RA. The total number of bacteria was calculated by using direct culture in serial dilution by applying the total plate count method. Isolation of microbiota was conducted by pour plates and incubated in an anaerobic jar. The bacterial diversity was performed by Simpson index and DNA Isolation. Results: There were differences in the concentration of sub-gingival fluids bacteria in the control group with the periodontitis and RA group. The microbiota composition profiles were different for each group. The bacterial isolate 4, 5 and 22 were found in all groups, while isolate 14 was assumed to be related to the metabolic pathway in microbiota abnormalities associated with disease progression. Conclusion: The Simpson index has a value >0.61 with high bacterial dominance; however, the diversity of microbiota is at a low level of diversity. We assumed that isolate 14 was associated with development of RA disease.

ABSTRACT

Abstract Since December 2019, an outbreak of Coronavirus Disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) emerged in China and is now becoming a global Pandemic, with over 10.5 million cases worldwide. Angiotensin-converting enzyme-2 (ACE-2) has been considered the main receptor for the SARS-CoV-2 entry into human cells, and they are known to be present not only in lungs (therefore the common viral pneumonia) but also in nasopharyngeal mucosa, salivary cells and oral epithelial cells. Moreover, there seems to be recent evidence showing that the crevicular fluid on the periodontal pockets of patients presenting periodontitis could harbor SARS-CoV-2 and act as a potential reservoir for increased viral load in the oral cavity of COVID-positive patients. Additionally, sites with active periodontal disease might contribute for virus binding and tissue infection due to elevated expression of furin and cathepsin L proteases, which play a major role in enabling the SARS-CoV-2 to bind ACE-2 receptors and facilitating endosomal fusion in the host cells. Taken together this news and views article highlight possible virus reservoirs in COVID-positive patients, as well as increased risk for infection in patients with active periodontal disease, further proposing potential targets for antiviral intervention on SARS-CoV-2 infection.

Abstract Objective: To analyze the differences in periodontal severity between perimenopausal and postmenopausal women with chronic periodontitis. Material and Methods: A cross-sectional study with 63 subjects, comprising 27 perimenopausal and 36 postmenopausal women, aged 45-59 years, was conducted in East Jakarta. The women were interviewed regarding their perimenopausal and postmenopausal status; they underwent a periodontal examination for periodontal pockets, attachment loss, gingival recession, plaque index, debris index, calculus index, oral hygiene index, papilla bleeding index, and tooth mobility. Independent T-test and Mann-Whitney U test were used for bivariate analysis Results: There were significant differences (p<0.05) in age between perimenopausal and postmenopausal women (p=0.01); however, no significant differences (p>0.05) in gingival recession (p=0.33) or tooth mobility (p=0.84) were observed. Independent t-test revealed no significant differences in pocket depth (p=0.95), attachment loss (p=0.71), plaque index (p=0.89), debris index (p=0.52), calculus index (p=0.46), oral hygiene index (p=0.48), or papilla bleeding index (p=0.63) between perimenopausal and postmenopausal women Conclusion: There was no difference in periodontal severity between perimenopausal and postmenopausal women; however, the current study obtained valuable information regarding periodontal severity in perimenopausal and postmenopausal women.

Abstract Studies on the oral health of individuals with intellectual disability (ID) have identified problems that include a high prevalence of periodontal disease. The use of probiotics to treat periodontal disease has been the focus of considerable research, and bovine milk fermented with Lactobacillus rhamnosus L8020 (L8020 yogurt) has been shown to reduce the oral prevalence of four periodontal pathogens. Objective The aim of this randomized, double-blind, placebo-controlled trial was to compare the effects of L8020 yogurt (test group) with those of placebo yogurt (placebo group) on the papillary-marginal-attached (PMA) index, gingival index (GI), and probing depth (PD) in 23 individuals with ID. Methodology All patients were required to consume the allocated yogurt after breakfast for 90 days. PMA index and GI scores as well as PDs were assessed before the start of yogurt consumption (baseline), after 45 and 90 days of consumption, and 30 days after the cessation of consumption. Student's t-test, Mann-Whitney U test or Fisher's exact test was used for inter-group comparisons, and the mixed effect model of repeated measurements was used for data analysis. Results The decrease in PMA index score was significantly greater in the test group than in the placebo group (p<0.001). The GI score also decreased during the study, with a tendency for greater decrease in the test group. Furthermore, decreases in PD between baseline, 45 and 90 days tended to be greater in the test group than in the placebo group. Conclusion These results suggest that regular consumption of bovine milk fermented with L. rhamnosus L8020 can lower the risk of periodontal disease in individuals with ID.

Abstract There are few studies on the clinical and immunological periodontal status of intensive care unit (ICU) in-patients. The aim of the present study was to evaluate the periodontal condition among ICU in-patients through clinical and immunological periodontal parameters. From the sample of 373 hospitalized ICU patients, 182 were submitted' to a thorough clinical periodontal and immunological evaluation. Data on bleeding on probing (BOP), probing depth (PD), and clinical attachment level (CAL) were collected and gingival sulcular fluid samples were quantified through ELISA on IL-1, IL-6, and MMP-2 for immunological evaluation. Data was statistically analyzed by Chi-square, Fisher's exact, Mann-Whitney tests, and Sperman's correlation and multivariate logistic regression analysis. A high dental plaque index and a high prevalence of periodontitis (48.3%), mostly in moderate and localized chronic form, were observed. Individuals with periodontitis presented higher levels of IL-1 and MMP-2, while individuals with cardiovascular disease (CVD) and individuals with two or more systemic diseases (MSD) presented higher levels of IL-1; diabetes mellitus (DM) and MSD individuals presented higher levels of IL-6. A positive association was found between the severity of periodontitis and CVD (OR 2.2; CI = 1.11-4.42). This study reported a 48.3% of the prevalence of periodontitis in ICU patients and a positive association between the severity of periodontitis and CVD. Additionally, higher levels of IL-1 and MMP-2 were found in individuals with periodontitis, higher levels of IL-6 were found in individuals with DM, and higher levels of IL-1 were found in individuals with CVD.


Resumo Existem poucos estudos sobre o estado clínico periodontal e imunológico de pacientes em unidade de terapia intensiva (UTI). O objetivo do presente estudo foi avaliar a condição periodontal entre os pacientes internados na UTI através de parâmetros clínicos periodontais e imunológicos. De uma amostra inicial de 373 pacientes internados em UTI, 183 foram submetidos a exame periodontal completo e análise imunológica. Os dados sobre o sangramento na sondagem (BOP), profundidade de sondagem (PD) e nível clínico de inserção (CAL) foram coletados e as amostras de fluido sulcular gengival foram quantificadas para avaliação imunológica através de ELISA para IL-1, IL-6 e MMP-2. Os dados foram analisados estatisticamente pelos testes de Qui-quadrado, exato de Fischer, Mann-Whitney, correlação de Sperman e análise de regressão logística multivariada. Foi observado um alto índice de placa dental e uma alta prevalência de periodontite (48,3%), principalmente na forma crônica moderada e localizada. Os indivíduos com periodontite apresentaram níveis mais altos de IL-1 e MMP-2, enquanto indivíduos com doença cardiovascular (CVD) e com mais de duas doenças sistêmicas (MSD) apresentaram níveis mais altos de IL-1 e os com diabetes mellitus (DM) e MSD apresentaram níveis mais elevados de IL-6. Foi encontrada associação positiva entre a gravidade da periodontite e CVD (OR 2.2; IC = 1,11-4,42). Este estudo reportou uma prevalência de periodontite em 48.3% dos pacientes em UTI e uma associação positiva entre ocorrência de periodontite e CVD. Além disso, níveis mais elevados de IL-1 e MMP-2 foram encontrados em indivíduos com periodontite, de IL-6 em indivíduos com DM e de IL-1 em indivíduos com CVD.

ABSTRACT

Introducción: la periodontitis y la infección subgingival anaerobia asociada representan injuria metastásica que disemina mediadores inflamatorios favorecedores de la ateroesclerosis. La alta tasa de mortalidad por enfermedades cardíacas y cerebrovasculares en Cuba demanda estudios sobre factores de riesgo emergentes, para dirigir esfuerzos a su prevención y control. Objetivo: caracterizar la asociación entre enfermedad periodontal inmunoinflamatoria crónica (EPIC) y un grupo de enfermedades derivadas de la aterosclerosis. Materiales y Métodos: entre enero y marzo del 2017 se realizó un estudio transversal descriptivo, de casos y controles, en el consultorio No 13 del Policlínico "19 de Abril", municipio Plaza de la Revolución. Se conformó el grupo de casos con pacientes que padecieron enfermedades seleccionadas derivadas de la ateroesclerosis; el grupo de controles con sujetos que no las habían presentado. Se homologaron según edad, sexo y color de la piel. Se exploraron factores de riesgo ateroescleróticos y prevalencia y severidad de la EPIC según el Índice Periodontal Revisado. Se respetaron normativas bioéticas para investigaciones biomédicas. Se emplearon medidas de resumen para datos cualitativos y la prueba de independencia de chi-cuadrado (x2) para variables cualitativas. Resultados: existió igual cantidad de hombres que de mujeres, mayoritariamente blancos, entre 61 y 70 años. Las complicaciones ateroescleróticas más frecuentes fueron la angina de pecho y el infarto del miocardio. El factor de riesgo más común fue la hipertensión arterial. Solo hubo 3 pacientes sanos según la Periodoncia. Conclusiones: la asociación entre la EPIC y el grupo de enfermedades estudiadas derivadas de la aterosclerosis no fue estadísticamente significativa (AU).


Introduction: Periodontitis and its associated anaerobic subgingival infection represent metastatic injury that disseminates inflammatory mediators, which favor atherosclerosis. The high mortality rate for heart and cerebrovascular diseases in Cuba requires studies on emerging risk factors to direct efforts to their control and prevention. Objective: To characterize the association between chronic immune inflammatory periodontal disease (CIIPD) and some entities derived from atherothrombotic disease. Materials and Methods: Between January and March 2017, a cross - sectional descriptive study of cases and controls was carried out in the Medical Office # 13 of the Policlinic "19 de Abril", municipality of Plaza de la Revolución. The cases group was formed with patients who have suffered the chosen diseases derived from arteriosclerosis. The control group was formed by 1 X 1 mating, with subjects who had not presented diseases derived from atherosclerosis. They were validated according to age, sex and skin color. Atherosclerotic risk factors were explored, and also the prevalence and severity of CIIPD according to the Revised Periodontal Index. Bioethical principles and regulations for biomedical research were fulfilled. Summary measures were used for qualitative data and independent chi square test (X2) for qualitative variables. Results: The amount of men and women was the same, mostly white persons, aged 61-70 years. The most frequent atherosclerotic complications were angina pectoris and myocardial infarction. The most common risk factor was hypertension. From the point of view of Periodontics, there were only 3 healthy patients. Conclusions: The association among CIIPD and the chosen diseases derived from arteriosclerosis was not statiscally significant (AU).

Introducción: la encía marginal, encía adherida, mucosa alveolar y unión mucogingival conforman el complejo mucogingival y las aberraciones adquiridas de la morfología, la posición y la cantidad de gingiva que rodea los dientes constituyen los trastornos mucogingivales. Objetivo: determinar el comportamiento de los trastornos mucogingivales y su tratamiento en el servicio de Periodoncia de la Clínica Estomatológica Docente “III Congreso del PCC” en Matanzas, entre enero 2008 y diciembre 2012. Materiales y métodos: se realizó un estudio observacional, descriptivo, en una población de 329 pacientes de ambos sexos. Resultados: la periodontitis y la recesión periodontal resultaron las afecciones periodontales más frecuentes, con un 58,3 y 26,9 %, respectivamente. La recesión periodontal fue el trastorno mucogingival predominante y se presentó con mayor frecuencia en mujeres entre 19 y 34 años de edad. De los 51 pacientes remitidos de ortodoncia que se mantenían en seguimiento evolutivo periodontal la mayoría, 42 (82,4 %), pertenecían al grupo de edad de 5 a 11 años. Los pacientes entre 35-59 años fueron los más afectados por periimplantitis en ambos sexos. Conclusiones: la periodontitis y la recesión periodontal resultaron las afecciones periodontales mayormente notificadas. La recesión periodontal y los trastornos mucogingivales asociados a zonas implantadas fueron más frecuentes en el sexo femenino. La mayoría de los pacientes remitidos de ortodoncia, estaban en el grupo de 5-11 años y se mantenían bajo control evolutivo periodontal. La técnica de cirugía mucogingival más utilizada fue el injerto libre de tejido conectivo y epitelio en todas las edades (AU).

Objective: to determine the behavior of the mucogingival dysfunctions and their treatment in the service of Periodontology of Odontology Clinical "III Congress of PCC" in Matanzas, among January 2008 and December 2012. Materials and methods: it was carried out an observational, descriptive study, in a population of 329 patients of both sexes. Results: Periodontitis and the periodontal recession were the most frequent periodontal affections, with a 58,3 % and 26,9 %, respectively. The periodontal recession was the mucogingival dysfunction predominant and it showed up most often in women among 19 and 34 years of age. Of the 51 remitted patients of Orthodontics that stayed in evolutionary periodontal pursuit most, 42 (82,4 %), they belonged to the age group from 5 to 11 years. The patients among 35-59 years were those more affected by periimplantitis in both sexes. Conclusions: Periodontitis and the periodontal recession were the principal periodontal affections. The periodontal recession and the mucogingival dysfunctions associated to implanted areas, were more frequent in the female. Most of the remitted patients of Orthodontics, were in the 5-11 year old group and they stayed periodontal low evolutionary control. The mucogingival technique of surgery more used was the free autograft of connective tissue and epithelia in all the ages (AU).

Os mediadores químicos produzidos nas doenças periodontais podem gerar a antecipação das contrações uterinas, levando ao nascimento precoce do bebê e seu baixo peso. Objetivo: realizar uma revisão da literatura abordando a doença periodontal e seus fatores de riscos que se associam às complicações obstétricas, como o parto prematuro e/ou baixo peso ao nascer. Material e métodos: foram utilizadas as bases de dados Lilacs, PubMed e Scielo no período entre janeiro de 2014 e julho de 2014. A busca incluiu artigos publicados em periódicos, dissertações e teses, sem restrições de idioma. Conclusão: é possível que haja participação da doença periodontal como fator de risco ao parto prematuro e/ou baixo peso ao nascer, porém, através da literatura essa relação se mostrou inconclusiva.


The chemical mediators produced in the periodontal diseases, can generate the anticipation of the uterine contractions leading to the early birth of the baby and consequently its low weight. Objective: to review the literature addressing periodontal disease and its risk factors associated with obstetric complications such as preterm birth and/or low birth weight. Material and methods: Lilacs, PubMed, and Scielo databases were screenedbetween January 2014 and July 2014. The search included articles published in periodicals, dissertations and theses, without language restrictions. Conclusion: it is possible that periodontal disease maybe a risk factor for preterm birth and/or low birth weight, but this relationship was inconclusive through the literature.

ABSTRACT

Porphyromonas gingivalis (P.Gingivalis) es un microorganismo comprometido en el inicio y progresión de la enfermedad periodontal crónica y agresiva, y es considerado su principal agente etiológico. Esta bacteria cuenta con una serie de factores de virulencia que le permiten, iniciar el proceso infeccioso, perpetuar la infección y también transformar la placa dental benigna en una comunidad microbiana patógena. Estudiar sus factores de virulencia y su capacidad de modular la respuesta inmunológica del huésped es muy importante para comprender el papel de este patógeno en el desarrollo y establecimiento de la enfermedad. Esta revisión proporciona una visiónactual sobre los factores de virulencia y su impacto sobre la respuesta inmunológica en relación con la patogénesis de la enfermedad periodontal.

Introducción: toda enfermedad está basada en trastornos a nivel molecular, por lo que el profesional de la salud debe interpretar las variadas manifestaciones internas de las enfermedades producidas por alteraciones de las biomoléculas que interactúan sistémicamente, en las cuales tienen un papel importante el estrés oxidativo. Objetivo: realizar una revisión bibliográfica sobre el estrés oxidativo y las enfermedades que afectan a la cavidad bucal. Métodos: se realizó una revisión bibliográfica en el periodo comprendido entre septiembre de 2014 a enero de 2016. Se consultaron las bases de datos de sistemas referativos, como MEDLINE, PubMed y SciELO con la utilización de descriptores como oxidative stress in oral deseases y su contraparte en español. Se incluyeron artículos en idioma inglés y español y publicaciones de los últimos cinco años, solo seis con mayor tiempo de publicación. Se revisaron 110 artículos, y se circunscribió el estudio a 49 que enfocaron estas temáticas de manera más integral. Análisis e integración de los resultados: tiene el estrés oxidativo gran relación con la enfermedad periodontal, la mucositis, los estados pulpares, síndrome de Sjögren, síndrome de ardor bucal, aftas bucales y lesiones premalignas como leucoplasia y liquen plano bucal. Conclusiones: existe relación entre el estrés oxidativo y muchas enfermedades que afectan la cavidad bucal, lo que influye en la patogenia de estas.(AU)


Introduction: the basis of disease is the occurrence of disorders at the molecular level, so the health professional should be able to understand the varied internal manifestations of a disease caused by alterations in the biomolecules that systemically interact and the important role that the oxidative stress plays in this process. Objective: to make a literature review on the oxidative stress and the diseases affecting the oral cavity. Methods: a literature review was made in the period of September 2014 through January 2016. MEDLINE, PubMed and SciELO databases were consulted; the subject headings were oxidative stress in oral diseases in Spanish and in English. Several articles in English and Spanish and publications of the last five years, in addition to 6 articles published prior to this period, were all included. A total of 110 articles were reviewed, but for the study, 49 were selected on account of a more comprehensive approach on these topics. Data analysis and integration: the oxidative stress is closely related to periodontal disease, mucositis, pulpal diseases, Sjögren syndrome, burning mouth syndrome, oral aphthas and premalignant lesions such as leukoplakia and oral lichen planus. Conclusions: there is association of the oxidative stress and many other diseases affecting the oral cavity, which has an impact on the pathogenesis of many diseases(AU)
